However, the overall survival of patients with metastatic disease continues to be less than 30%4-10. Thus, there is an urgent need for new therapies which can improve outcomes in these patients3,9,10. Recent studies have shown that the transmembrane protein CD47 is a critical regulator of osteosarcoma growth and metastasis11. Therefore, inhibition of CD47 represents a new and promising therapeutic approach. CD47 expression on sarcoma cells functions as a “don’t eat me” signal by engaging signal-regulatory protein alpha (SIRPα), an inhibitory receptor on tumor associated macrophages (TAM)12,13. CD47 monoclonal antibodies (mAbs) block the interaction between CD47 and SIRPα, and thereby, activate TAM to phagocytose cancer cells14-18. Recent work in animal models of sarcomas showed that CD47 mAb therapy significantly reduced tumor growth and increased survival 19-21. However, tailoring CD47 mAb therapy for pediatric patients is difficult due to a lack of clinically established biomarkers, which can monitor TAM response in vivo. Since immunotherapy does not lead to a decrease in tumor size, at least not in the immediate post-treatment phase, we urgently need new quantitative imaging (QI) tools that can monitor tumor response to these novel therapies The overall goal of our project is to optimize and validate preclinical and clinical quantitative imaging techniques for in vivo quantification of tumor associated macrophages (TAM) in osteosarcomas. We are in a unique position to leverage an immediately clinically applicable TAM imaging test, which was developed in our lab, to study the efficacy of TAM-modulating therapies. Our approach relies on the FDA approved iron oxide nanoparticle compound ferumoxytol, which is phagocytosed by TAM and can be detected with magnetic resonance imaging (MRI). Our preliminary data provide proof-of-concept that ferumoxytol-MRI can detect TAM in osteosarcomas in mouse models21 and patients 22. We also showed that ferumoxytol-MRI can monitor tumor response to immune-modulating therapies in mouse models 23. To develop quantitative imaging (QI) methods for monitoring cancer therapy response to TAM activating cancer immunotherapy, we propose the following specific aims:

Aim 1

Optimize and validate pre-clinical quantitative imaging methods for TAM imaging

Our preliminary data showed a specific cellular uptake of ferumoxytol in TAM in osteosarcomas in mouse models and corresponding significant tumor enhancement on T2- and T2*-weighted MR images at 24 hours after intravenous nanoparticle injection21. With this aim, we will define the appropriate time window for TAM imaging, determine the reproducibility of quantitative TAM imaging results and identify potentially confounding signal effects. All imaging studies will be performed on a 3T MRI scanner, which resembles the typical clinical situation. The endpoint will be to validate a quantitative TAM imaging test in an established animal model.

Aim 2

Implement the optimized methods in a co-clinical trial

We recently conducted a first-in-human clinical trial, which showed that ferumoxytol nanoparticles cause strong MRI enhancement of bone sarcomas. A hypointense (dark) tumor enhancement on T2-weighted MRI images correlated with the presence and distribution of TAM on histology22. However, it is not known whether this new imaging test can be used to predict response to TAM-modulating therapies. We hypothesize that the degree of ferumoxytol tumor MRI enhancement after TAM-modulating therapy correlates with the quantity of TAM and the extent of tumor necrosis on histology. The endpoint will be to prove that quantitative ferumoxytol-enhanced MRI can serve as a surrogate outcome measure for tumor response to TAM modulating therapies.

Aim 3

Populate a web-accessible research resource

Finally, we will populate a web-accessible research resource with all the methods, data and results collected from the co-clinical investigations such that investigators at other centers can learn about quantitative imaging approaches for TAM imaging, test their ability to estimate tumor TAM content through R2 and R2* maps, implement related imaging protocols at their institution and apply them for monitoring TAM-modulating cancer immunotherapies.
